Contact us about insurance.

We are an in-network provider with a number of dental insurance providers such as Delta Dental, Cigna, Aetna, Blue Cross Blue Shield, United Concordia and Sun Life (Assurant). While not in-network, we accept many other insurances including, MetLife, UnitedHealthcare, Guardian, Principal and more.

Please note that we are changing our insurance policies in 2022, so please contact us before you make your appointment. We are happy help with your dental insurance needs

If you are insured by one of our accepted dental insurance providers, payment of all copays, deductibles, and/or coinsurance are due at the time of service. We accept cash, checks, MasterCard, Visa, American Express and Discover.

Please bring your insurance information with you to your first visit.

Health Screening

New and established patients should review the new health screening questions we will ask you beforehand and again when you are in the office.

New Patients

Before your first visit:

  1. Download our New Patient Packet.
  2. Fill out the Health History.
  3. Review our Privacy Practices — Sign the acknowledgement.

Bring with you to your Appointment (or email in advance):

  1. Completed Health History.
  2. Signed Privacy Acknowledgement
  3. Dental Insurance Card (if any).

If you prefer to complete the forms in our office, we will provide them to you at that time. Thanks.
